Colleague Change Notification Form
This form should be used to identify when a change has been made to a student's record. Fill in your DTCC email address, the Colleague ID # of student you are making a change to, and check the boxes that are applicable. This information will be distributed to all parties that need to make changes in their systems to match the change made on the NAE, RGPE, and BIO screens. This is to only be used for student changes. Any changes to employee records need to go through Human Resources.
