In order to request information from the Research, Evaluation, Assistance, and Planning Department, complete and submit an Assistance Request Form. To ensure timely processing, a department head should submit this form. Thorough completion of the form will ensure the best possible outcome for the request. The person completing this form should have access to the information requested or the permission of the department that has access.
